If you already have an appointment time, please bring the following items with you to your new patient visit:

• Completed registration information
• Medication bottles or a current list of medications with proper dosage and instructions
• Current insurance/Medicare/Medicaid cards
• Some form of picture identification
• Recent medical records from referring physicians and/or a previous cardiologist

The office is located at 1721 Birmingham Drive in College Station. From Rock Prairie Road, go south on Longmire Drive. The office is at the corner of Birmingham and Longmire, between the Kroger shopping center and the back of College Station Medical Center.
